One-on-one instruction focuses on daily living skills training and community socialization.

This service typically provides 4 to 10 hours of support each week.This service is provided to individuals living independently or in their natural homes.

Qualifications

  • DD diagnosis prior to age 22
  • Qualify for case management services

  • DMH general revenue, or any Medicaid Waiver*

*Department of Mental Health- Purchase of Service: a DMH funding source coming out of the state’s general revenue budget, which can be used for off-site day habilitation or personal assistant.
