Services

Sunnyhill provides tailored residential support to people diagnosed with developmental disabilities in various settings. Our residential services range from support a few hours a week to 24 hours a day.

Residential services can be delivered in a person’s natural home, a community-based home, or one of our residential facilities. All of our residential supports are provided by personnel trained in abuse and neglect, CPR/first aid, positive behavioral supports, medication administration, nutrition, and personal care.

The goals of our residential supports are  To foster independence, assist with skill acquisition and development, promote community participation and inclusion, develop social skills and relationships, provide a healthy and safe environment, and help the person participate in meaningful activities.

24-Hour Care

Sunnyhill’s community-based Independent Supported Living Medicaid Waiver program (Waiver) is designed to meet the support needs of each person served.

People receiving Waiver reside in a community home with one, two, or three roommates. The goal is to teach living skills and create opportunities for people to fulfill their potential and reach their dreams.

Each person has a support team with family, medical supports, employment, day programs, schools, community resources, case managers, and other natural supports. The support team works with each person to live independently in the community.

Sunnyhill support staff is available 24 hours a day to assist with instruction on daily living skills, participation in meaningful activities, relationship formation and community access. Sunnyhill currently provides Independent Supported Living services in 39 locations and counting.

  • Qualifying PON score

  • Demonstrate significant need

  • Medicaid eligible

ISLA Program

People in the Independent Supported Living Assistance (ISLA) program receive one-on-one service focusing on the maintenance of a person in his or her own residence.

Support Personnel promote the acquisition of skills for independence, formation of social roles, relationships and self-reliance in children and adults who participate in the program.

This service can include up to 40 hours of support a month. Sunnyhill currently supports over 256 people through this program.

Off-Site Day Habilitation

One-on-one instruction focuses on daily living skills training and community socialization.

This service typically provides 4 to 10 hours of support each week.This service is provided to people living independently or in their natural homes.

Behavioral Analysis Services

Behavioral Analysis Services are designed to teach daily living skills that are socially significant and abate maladaptive behaviors that negatively impact an person and natural supports. Behavior Analysis Services can include completion of a functional behavioral assessment, development of behavior support plans, skill acquisition programs (functional communication skills and daily living skills), and training of paid/natural supports.

All services will be provided by a Licensed Behavior Analyst.
